Overview

We are seeking a Case Manager to coordinate and oversee end-to-end case management within a fast-paced, regulated environment. You will act as a key point of contact for clients and internal stakeholders, ensuring timely, compliant and high-quality outcomes, while maintaining accurate records and safeguarding confidential information.

Key Responsibilities

  • Manage a caseload from intake to closure, including assessment, planning, implementation, monitoring and review.
  • Conduct needs assessments, identify risks, and develop structured case plans with clear goals and timelines.
  • Liaise with internal teams and external partners to coordinate services, referrals and follow-ups.
  • Maintain comprehensive case notes, documentation and reporting in line with policy, audit and data protection requirements.
  • Monitor progress against outcomes, resolve barriers, and escalate complex or high-risk cases appropriately.
  • Ensure compliance with relevant legislation, safeguarding standards, organisational procedures and ethical practice.
  • Communicate clearly with clients, providing guidance, updates and managing expectations with sensitivity.
  • Contribute to service improvement through feedback, trend analysis and participation in quality assurance activities.

Requirements

  • Proven experience in case management, care coordination, claims handling, social services, healthcare, insurance, legal support, or a similar case-based role.
  • Relevant qualification in social work, psychology, nursing, counselling, community services, health sciences, law, or equivalent practical experience.
  • Strong understanding of risk assessment, safeguarding, confidentiality, consent and professional boundaries.
  • Excellent written and verbal communication skills, with the ability to produce clear reports and maintain accurate records.
  • Strong organisational skills, able to prioritise competing deadlines and manage a high-volume caseload.
  • Competence with case management systems, Microsoft Office and accurate data entry.
  • Ability to work independently and collaboratively, demonstrating sound judgement and attention to detail.
  • Willingness to undergo background screening and comply with relevant regulatory checks (where applicable).
